Dominic Chavez, Community Liaison

I am Dominic, a dedicated professional with a strong commitment to supporting others on their recovery journey. Raised in Milford, Utah, on a beef cattle and alfalfa farm, I come from a family with a deep-rooted tradition of healthcare service. My grandmother served as the Director of Nursing (DON) at Milford Valley Hospital for over 45 years, and her legacy inspires me to make a positive impact on people's lives.

My family is my greatest source of joy, including my five children—Anthony, Monty, Traysen, Dane, and Keylee—and my two cherished granddaughters, Evey and Sky. In my personal time, I enjoy activities such as golf, bowling, following the 49ers, and deer hunting.

It is a privilege and an honor to be part of this remarkable team, where we work tirelessly to assist individuals in their recovery, enabling them to embrace life to its fullest potential.
